The 2022 Community Health Needs Assessment (CHNA) — developed over 18 months — details how social, economic, and environmental factors shape health outcomes for residents.
The assessment shows substantial disparities in health conditions across age, race, ethnicity, and education level. Data further reveal a strong correlation between ethnicity and food insecurity, with some ethnic groups facing greater challenges in accessing nutritious food — a factor that contributes to wider health inequities.
The CHNA was developed with input from community members and key stakeholders, including local government officials, healthcare providers, and nonprofit organizations. Their participation helped identify priority issues and propose strategies to improve health outcomes.
The report calls for a proactive, multi-sector approach to address root causes of health disparities.
Recommended actions include expanding access to healthcare, improving educational and economic opportunities, and upgrading living conditions in underserved communities.
